What Is Going On

Outside the hip or inside the joint.

Pain on the outer hip, the side you cannot lie on at night, is most often gluteal tendinopathy with an irritated bursa over it. Like all tendinopathy it is a stalled repair process in tissue that has been overloaded, not a simple inflammation, which is why rest and a cortisone shot calm it and it returns.

Pain deep in the groin, especially with stiffness turning the leg or getting out of a car, is more typically the hip joint itself. That is arthritis territory, and the treatment shifts toward the joint, the capsule, and the muscles controlling how it loads.

The back complicates both. The low back and sacroiliac joint refer pain straight into the hip and buttock, which is why some hip pain does not improve until the back is treated. Sorting that out is part of the exam.

How We Treat It

Four tools, matched to your case.

No single therapy fits every hip. These are the four we use. Which ones you get, and in what combination, is decided at your exam.

Shockwave therapy

For the tendon side of hip pain. Acoustic pressure waves delivered to the gluteal tendons restart the remodeling process in tissue that has stopped repairing, and it has good support for greater trochanteric pain. How shockwave works.

Acupuncture and dry needling

Needling releases the deep hip rotators and gluteal muscles that load the joint and tendons, and calms the pain signaling that keeps the hip guarded. How acupuncture works.

Laser therapy

Low level laser reduces inflammation in the bursa and joint lining without any mechanical stress, useful when the hip is too irritable to work firmly. How laser works.

Acupuncture injections

Anti inflammatory and regenerative injectables placed at points around the tendon and joint, used when the tissue needs more support than needling alone provides. How injection therapy works.

What To Expect

A typical plan.

01

We locate the real source

Outer hip tendon, hip joint, and referred pain from the low back all present as hip pain and often overlap. We examine the hip, the back, and your history before treating anything.

02

We choose the therapies

Shockwave, acupuncture, laser, and injections all treat hip pain. Which ones lead depends on whether the problem is tendon or joint, and we will explain the reasoning.

03

You keep moving, with guidance

Hips do badly with rest, and side sleeping and stair patterns matter. We tell you what to keep doing, what to change, and what supports the treatment between visits.

04

We check progress honestly

Night pain on that side, stairs, and walking distance are easy to track. If the plan is not working, we adjust it or tell you when a surgical opinion is warranted.

Is This You

A strong fit for this care.

This approach tends to work best when:

  • The hip pain has been present for more than three months.
  • You cannot lie on that side at night, or stairs and getting out of the car are the worst.
  • A cortisone shot or rest has helped temporarily and it returned.
  • Imaging, if you have had it, showed tendinopathy, bursitis, or moderate arthritis.
  • You are trying to stay active, or to delay a hip replacement.

When it is something else. Sudden severe groin pain after a fall, or inability to bear weight, needs imaging to rule out a fracture, particularly if you have osteoporosis. Hip pain with fever or a hot swollen joint needs urgent care. We screen for both at the consultation.

Cost, Insurance and Visits

Hip Pain Treatment cost and insurance in The Villages.

What sets the cost

Hip pain plans combine acupuncture with shockwave for gluteal tendons, laser, or point injections. Each therapy is priced on its own.

Insurance

It depends on the therapy. Acupuncture visits are covered by many insurance plans and we check yours before the first appointment. Shockwave, laser, injections, peptides and NAD+ are self pay, and you will know the price of each before you agree to it.

How many visits

Hips usually run weekly for four to six visits, then taper. Night pain on the side often improves first.

Your first 20 minute consultation is free, and no referral is needed. Call 352-430-2720. Insurance and payment details.

Common Questions

Hip Pain, answered.

Side lying compresses the gluteal tendons and the bursa directly against the bone at the outside of the hip. Night pain on the affected side is the classic sign of greater trochanteric pain, and it easing is usually the first sign treatment is working.
Sometimes, and it is one of the most common reasons hip treatment fails elsewhere. The low back and sacroiliac joint refer into the buttock and outer hip. The exam distinguishes them, and when both are involved we treat both.
That depends on the joint and how much of your life the pain is taking. Many outer hip problems have nothing to do with the joint at all and never need surgery. If your joint genuinely needs replacing, we will say so.
Because it calmed the inflammation without rebuilding the tendon underneath, and the tendon was the actual problem. Restarting repair in that tissue is a different goal, which is what shockwave is for.
Usually yes, with adjustments to volume. Total rest deconditions the tendon and the muscle around the joint, which sets up the next flare. We give specific guidance rather than a blanket stop.
That depends on how long the pain has been there, how the tissue looks on exam, and which therapies we use. Some people need a short series, others need longer. Dr. Cheshire will give you a specific plan at your consultation rather than a number off a page.
In most cases yes, with adjustments. Tissue rebuilds in response to load, so total rest usually is not the answer. We will tell you what to keep doing, what to scale back, and for how long.
Coverage depends on the therapy and your plan.

In The Treatment Room

Working the hip and the chain around it.

Hip treatment here works the tendons on the outside, the deep rotators, and the low back that refers into it, a few sites per visit, usually across a short series of visits.
