To make your first visit as smooth as possible, we encourage you to download and complete our new patient forms before your appointment. This allows Dr. Cheshire to review your medical history in advance and make the most of your consultation time.
Please bring your completed patient forms, a valid photo ID, your insurance card if applicable, and a list of any current medications and supplements. If you have recent lab work or imaging results relevant to your condition, please bring those as well.
During your initial consultation, Dr. Cheshire will review your health history, discuss your symptoms and goals, perform an assessment, and develop a personalized treatment plan. We take the time to listen and answer all of your questions so you feel confident in your care plan.
