The Trial Record
What has been tested on knees, and what the numbers were.
Acupuncture for knee osteoarthritis
A 2024 systematic review pooled ten randomized trials with 3,221 participants Source: Chen et al., Current Pain and Headache Reports, 2024 (10 trials, 3,221 participants). Against sham needling, acupuncture improved pain and function, and the benefit was still measurable about four and a half months after treatment ended. A separate trial of more than 400 patients compared electroacupuncture, manual acupuncture and sham: response rates at eight weeks were 60 percent, 59 percent and 47 percent. Real, modest, and the best-supported non-surgical option short of exercise.
Low level laser
Multiple randomized trials in knee osteoarthritis report reduced pain and better function with laser against placebo light, with effects depending on dose. Smaller than the acupuncture literature, consistent enough that we use it on the inflamed lining, painlessly, alongside needling.
Shockwave for the kneecap tendon
Patellar tendinopathy has randomized evidence for shockwave improving pain and function where the tendon has stalled. It is not a treatment for the arthritic joint itself, which is why the exam sorts tendon from joint before anything is scheduled.
What has no trial behind it
The 80 percent pain-free figures advertised for some regenerative and device treatments around The Villages. No published trial produces a number like that for any knee treatment, including surgery. Ask where it came from.
What We Do With It
Match the tool to the knee, then measure the knee.
Most knee pain in adults over fifty is arthritis, a worn tendon, or both. The joint lining, the tendons above and below the kneecap, and the thigh and hip muscles that control the loading all contribute, and the pain tracks with them rather than with the x-ray. An arthritic knee gets needling and laser together; a tendon knee gets shockwave; a joint that will not hold gets non-steroid point injections. Nothing regrows cartilage, and a knee that locks, gives way or stops responding is referred to an orthopedic surgeon, with Dr. Phipps, MD, in the building.
Then the part no trial can do for you: stairs, walking distance and swelling recorded at the first visit and compared at a checkpoint, so the plan is judged on your knee rather than on a pooled average. Full knee pain page.
What To Expect
What happens at the first knee pain treatment visit for Stonecrest patients?
Exam and baseline
Arthritis, tendon, meniscus or referred hip. Dr. Cheshire examines the knee and the chain above it, reads your imaging, and records the three markers.
Weekly for four to six visits
Needling and laser in one appointment for an arthritic knee; shockwave for a tendon; injections only if the joint will not hold. Twelve to fifteen minutes from the gate, early slots, free parking.
Keep loading it
Knees do worse with total rest. A walking volume and thigh strength work, which the evidence supports more than anything we do in the room.
Checkpoint
Markers against baseline. Working: stretch out. Not working: a different plan or an honest conversation about the surgeon.
