The Plan

What happens at the first back pain treatment visit for Stonecrest patients?

01

Visit one: the exam and the baseline

Dr. Cheshire examines the back and the hips, goes through your history and any imaging, and sorts joint, disc, muscle and hip driven pain, which each behave differently. Three markers are recorded: how long the morning stiffness lasts, how long you can sit, how far you can walk. Treatment usually starts the same visit.

02

Visits two to four: the tissue and the guarding

Weekly. Acupuncture and dry needling release the muscles that will not stop guarding and unload the irritated segment; cupping decompresses the fascia across the low back and hips. Laser or point injections are added only where the exam called for them. You leave each visit with what to keep doing and what to change about sitting and lifting.

03

Visit four or five: the checkpoint

The three markers compared with the baseline. Most chronic backs are clearly moving by now. If yours is not, we change the plan or tell you plainly what we recommend instead, including a referral with Dr. Phipps, MD, in the building.

04

Visits six to eight, then spacing out

Weekly until the markers hold, then every two weeks, then as needed. A flare later on is usually one or two visits, not a new series.

Why That Plan

Rarely one broken part. Usually three things at once.

The low back is a stack of joints, discs and thick layers of muscle and fascia. Chronic pain there usually comes from several at once: irritated joints, deconditioned deep stabilizers, tight fascia through the hips, and muscles that have been guarding so long they never release. Imaging so often disappoints because disc bulges and arthritis show up on the scans of people with no pain; what tracks with symptoms is how the tissue is loaded and how sensitized the signalling has become.

That is why the plan treats the tissue and the guarding together. Releasing one without settling the other is why relief does not hold, and why a single therapy rarely fixes a chronic back. Full back pain page.

The Four Therapies

Matched to your back at the exam.

Acupuncture and dry needling

The best studied of our tools for chronic low back pain. A pooled analysis of 29 randomized trials and roughly 18,000 patients found acupuncture beat sham needling and no acupuncture for back and neck pain Source: Vickers et al., Archives of Internal Medicine, 2012 (29 trials, 17,922 patients), with the benefit persisting after treatment. How acupuncture works.

Cupping

Decompresses the fascia across the low back and hips and restores circulation through tissue that has been tight for years. How cupping works.

Acupuncture point injections

Anti-inflammatory and regenerative injectables along the irritated segment when the tissue needs more than needling. Not cortisone. Injection therapy.

Low level laser

Reduces inflammation with no mechanical stress, for a back too irritable for anything firmer. How laser works.

Is This You, Stonecrest?

The plan fits when:

  • The back pain has lasted more than three months or keeps returning.
  • Imaging showed nothing that requires surgery, or you have not needed imaging.
  • It eases with movement and worsens with prolonged sitting or standing.
  • You want a defined number of visits and a checkpoint, not an open-ended plan.
  • You want to stay active rather than oorganize your life around it.

A physician first for bladder or bowel changes, groin numbness, worsening leg weakness, fever, unexplained weight loss or a cancer history. We screen for all of it at the consultation.

Common Questions

What do Stonecrest patients ask about back pain treatment?

An exam that sorts joint, disc, muscle and hip, then acupuncture and dry needling, cupping, and laser or point injections where needed, weekly for four to eight weeks, with a checkpoint at visit four or five.
A flare often settles in a few. A long-standing back usually needs weekly visits for four to eight weeks before spacing out. Set after the exam. Progress is scored against measurements taken at the first visit, and the plan is changed or stopped if the numbers have not moved by the checkpoint.
Morning stiffness, sitting tolerance and walking distance, recorded at baseline and compared at the checkpoint. We do not assume. We state the limits of the evidence as plainly as the results, and measure your own progress from the first visit rather than quoting success rates.
Usually not. Findings often do not match symptoms. If something needs imaging, we say so.
