Rotator Cuff Treatment for Del Webb Spruce Creek, FL

Spruce Creek residents who search it have usually read a chiropractor's "amazing results" and a surgeon's "unproven," and both skip the trials. This page states what the randomized trials found for a worn cuff, what they found for a calcific one, where the evidence is thin, and what we do with that when your shoulder walks in. The Trial Record

Two different answers, for two different cuffs.

The evidence for shockwave in the shoulder splits cleanly by whether there is a calcium deposit in the tendon. Honest pages say which one they are quoting.

Calcific tendinitis: strong, short term

Where a calcium deposit has formed in a worn cuff tendon, high-energy focused shockwave has the strongest trial record of any shockwave use in the shoulder: multiple randomized trials and reviews find better resorption of the deposit and better pain and function than sham Source: Sun et al., Medicine, 2017 (9 placebo-controlled trials, 935 patients), at least in the months after treatment. This is the case shockwave was developed for.

Non-calcific tendinopathy: positive, but limited

For the ordinary worn cuff with no deposit, a 2024 review pooled seventeen randomized trials Source: Xue et al., BMC Musculoskeletal Disorders, 2024 (17 trials, about 1,100 patients), about 1,100 patients Source: Xue et al., BMC Musculoskeletal Disorders, 2024 (17 trials, about 1,100 patients), and found shockwave reduced pain and improved function compared with control. The authors rated the evidence limited: only five of the seventeen trials were at low risk of bias, doses and devices varied, and an earlier Cochrane review had called the evidence uncertain outside calcific cases. A real signal, not a settled one.

What no trial supports

A "pain free" percentage you could promise a patient; permanence beyond the follow-up windows, which mostly ran three to twelve months; and shockwave on a full-thickness tear, which was excluded from the trials and is a surgical question. We quote those limits because leaving them off would be marketing.

What it means for your shoulder

Shockwave is the best-supported tool for a chronic or calcific cuff that has outlasted rest, physical therapy and cortisone, with a modest real advantage over sham for the ordinary worn cuff and a stronger one for calcific. It is uncomfortable for a few minutes and it needs a series. We track your own nights and reach and tell you honestly if yours is not responding.

Why It Works On A Worn Cuff

Degeneration, not inflammation.

After fifty, most painful cuffs are worn rather than torn: micro damage has outrun repair, the tendon has thickened, the collagen has become disorganised and the repair has stalled, which is why rest and anti-inflammatories plateau. Shockwave delivers acoustic pressure waves into the tendon, breaks up the disorganised tissue and any deposit, and restarts the remodelling, which is why it needs a weekly series of about six rather than one session.

Around it, with less trial weight and stated as such: low level laser raises cellular energy in the tendon without mechanical stress for the irritable weeks; dry needling releases the neck, shoulder blade and chest muscles guarding the joint, often half the pain, and is one of the better studied uses of acupuncture for pain; point injections, not cortisone, support the tendon when it needs more. The exam decides which lead. Full rotator cuff page.

What To Expect

What happens at the first rotator cuff treatment visit for Del Webb Spruce Creek patients?

01

Confirm it is the cuff, and which kind

The trials were done on confirmed tendinopathy and calcific tendinitis. Dr. Cheshire examines the shoulder and the neck, tests strength, reads your imaging, and sorts worn from torn, calcific from not, cuff from frozen shoulder.

02

Weekly for about six visits

Shockwave and needling in one appointment, laser where the shoulder is irritable, injections only if the tendon needs more. Two therapies per drive from Spruce Creek.

03

Load between visits

Tendon rebuilds under load. A short home program, what to keep doing and what to scale back.

04

Nights and reach scored at every visit

Then a taper. If the shoulder is not turning by visit four we re-examine and say so. No success rate quoted, your own numbers instead.

Is This You, Spruce Creek?

A strong fit when:

  • The shoulder has hurt for more than three months.
  • Imaging showed tendinopathy, a calcium deposit or a partial tear, not a full rupture.
  • You want the evidence stated plainly before you spend anything.
  • It wakes you at night or decides how you sleep.
  • Cortisone or physical therapy helped for a while and the pain returned.

A surgical opinion first for sudden weakness after a fall or injury. Something else if the shoulder is stiff in every direction, or the pain runs from the neck down the arm. We screen for all three at the consultation.

Common Questions

What do Del Webb Spruce Creek patients ask about rotator cuff treatment?

For a calcific cuff, strong short-term trial support. For an ordinary worn cuff, a 2024 review of seventeen trials, about 1,100 patients, found less pain and better function against control, with the evidence rated limited. A real signal, quoted with its limits. Source: Xue et al., BMC Musculoskeletal Disorders, 2024 (17 trials, about 1,100 patients).
No trial supports a single percentage and we do not quote one. We score your nights and reach at every visit instead.
The deposit is a visible target that focused shockwave fragments, and the trials could measure it resorbing. Worn tendon without a deposit changes more slowly and the trials were smaller and more varied.
Trial follow-up mostly ran three to twelve months with benefit holding. Beyond that the data thins, and the home loading is what keeps a rebuilt tendon rebuilt. Progress is scored against measurements taken at the first visit, and the plan is changed or stopped if the numbers have not moved by the checkpoint.
Across the trials, mild soreness or bruising and no serious adverse events. It is uncomfortable for a few minutes, with no downtime. Not used on a full-thickness tear. Dr. Cheshire reviews your medications, including blood thinners, before the first treatment, and anything injectable is supervised by Dr. Phipps, MD.
About six, weekly, with home loading between, then a taper.
