Rotator Cuff Treatment for Oxford, FL
"Calcium deposit in shoulder" is what Oxford types after the x-ray report said "calcific tendinitis of the supraspinatus." It is a specific thing: a chalky deposit that forms inside a worn cuff tendon, can sit quietly for years, and can flare into some of the worst shoulder pain there is when the body starts to break it down. It is also the one rotator cuff problem where shockwave has its strongest trial record Source: Sun et al., Medicine, 2017 (9 placebo-controlled trials, 935 patients), because the pressure waves fragment the deposit as well as restarting the tendon around it. What A Calcium Deposit Is
Chalk in the tendon, and why it hurts in phases.
Calcific tendinitis is a deposit of calcium crystals that forms inside a rotator cuff tendon, most often the supraspinatus on the top of the shoulder, in a tendon that was already worn. Why it forms is not fully settled. What is settled is that it runs in phases. While the deposit is forming it is often painless or a dull ache with overhead reach. When the body begins to resorb it, the tendon swells, pressure inside it climbs, and the shoulder can become acutely, severely painful for days to weeks, worse at night, too sore to move. Then it can settle, with the deposit smaller or gone, or the cycle can stall part way and leave a chronic, sore, catching shoulder.
That is why the same x-ray finding means different things in different people, and why the exam matters more than the report. A quiet deposit in a shoulder that hurts for another reason is not the target. A deposit in a stalled, chronically painful cuff is, and it is the case shockwave was first developed for. Acute flares that are hot, swollen and unbearable need urgent assessment, and a few deposits that will not break down are removed surgically. We screen for both, and with Dr. Phipps, MD, in the building, we refer the same week when the picture fits.
How We Treat A Calcific Cuff
Shockwave on the deposit, and the three tools around it.
Shockwave: the lead, with the strongest record
Focused acoustic pressure waves aimed at the deposit fragment it and restart the remodelling of the tendon around it. For calcific tendinitis, high-energy focused shockwave has strong trial support in the short term Source: Sun et al., Medicine, 2017 (9 placebo-controlled trials, 935 patients), with reviews finding better deposit resorption and better pain and function than sham. A weekly series, uncomfortable for a few minutes, no downtime. How shockwave works.
Laser: when it is too sore for anything firmer
In a resorbing flare the shoulder will not tolerate pressure. Low level laser lowers inflammation and raises cellular energy in the tendon with no mechanical stress, and is the right first move in a bad week. How laser works.
Needling the guarding muscles
A calcific shoulder gets held still by the neck, shoulder blade and chest muscles, and that guarding outlasts the deposit. Dry needling releases it; acupuncture points calm the signalling. How acupuncture works.
Point injections
Anti-inflammatory and regenerative injectables at the points around the cuff when the tendon needs more than needling. Not cortisone. Injection therapy.
For non-calcific cuff tendinopathy the evidence is thinner and we say so: a 2024 review of seventeen randomized trials Source: Xue et al., BMC Musculoskeletal Disorders, 2024 (17 trials, about 1,100 patients), about 1,100 patients Source: Xue et al., BMC Musculoskeletal Disorders, 2024 (17 trials, about 1,100 patients), found shockwave reduced pain and improved function against control, with the evidence rated limited. We track your own night pain and reach.
What To Expect
What happens at the first rotator cuff treatment visit for Oxford patients?
7 AM exam: which phase, and is the deposit the cause
Dr. Cheshire examines the shoulder and neck, tests strength, reads your x-ray or ultrasound, and decides whether the deposit is the target or a bystander.
Weekly for about six visits
Shockwave on the deposit once the shoulder tolerates it, laser and needling first if it does not, injections only if the tendon needs more.
Load between visits
Tendon rebuilds under load. A short home program, what to keep doing and what to scale back during a flare.
Night pain and reach as the markers
A repeat image if we need to see the deposit. Then a taper. If it is not responding we say so, or say when removal is the right next step.
