Rotator Cuff Treatment for Wildwood, FL

"Shoulder pain at night" is searched at 3 AM, by someone who has just rolled onto the bad side again. Night pain is the signature of a worn rotator cuff: lying on the shoulder presses the irritated tendon against bone, and lying on the other side lets the arm hang and pull on it. If your shoulder decides which side of the bed you sleep on, the cuff is the likely cause. Rotator cuff treatment near Wildwood, FL starts with the nights: what to do with the pillows this week, then shockwave to rebuild the tendon, laser, needling for the neck and shoulder blade muscles guarding it, and point injections, by a board certified acupuncture physician with a medical doctor on site. Twelve to eighteen minutes via State Road 44 and County Road 466.

Why It Hurts At Night

Three reasons a cuff is worst lying down, and what to do this week.

None of this fixes the tendon. It buys sleep while the treatment does.

Compression on the bad side

Lying on the shoulder squeezes the swollen cuff tendon between the ball of the arm bone and the bony roof above it. Every turn in the night re-compresses it. For now: a pillow lengthwise behind your back so you cannot roll onto it, or sleep on the other side with the bad arm resting on a pillow in front of you, elbow bent, so it does not hang.

Traction on the good side

On the other side the bad arm drapes forward and down and pulls the tendon all night. Same fix: a pillow under the forearm keeps the arm level with the body. On your back, a small pillow under the elbow stops the shoulder dropping back.

Nothing to distract it

During the day the brain has other input; at night the sensitized signalling from a sore shoulder has the floor. A warm shower before bed loosens the guarding muscles, and the needling we do in clinic turns that signalling down for days at a time, which is usually the first change patients notice.

Fixing The Reason

A worn tendon, and the muscles clamped around it.

After fifty most painful cuffs are worn, not torn: micro damage has outrun repair, the tendon has thickened, the collagen has become disorganised and the repair has stalled, which is why rest and anti-inflammatories plateau. Around it, the neck, shoulder blade and chest muscles have tightened to protect the joint, and that guarding is a large share of the ache, day and night.

Shockwave delivers acoustic pressure waves into the cuff tendon, breaks up disorganised tissue and any calcific deposit, and restarts the remodelling. Low level laser raises cellular energy in the tendon without mechanical stress, the right first tool when a shoulder is too sore to lie on. Dry needling releases the guarding muscles and acupuncture points calm the signalling that keeps the shoulder loud at night. Point injections, not cortisone, support the tendon when it needs more. The evidence, with its limits: a 2024 review of seventeen randomized trials Source: Xue et al., BMC Musculoskeletal Disorders, 2024 (17 trials, about 1,100 patients), about 1,100 patients with rotator cuff tendinopathy Source: Xue et al., BMC Musculoskeletal Disorders, 2024 (17 trials, about 1,100 patients), found shockwave reduced pain and improved function against control, with the evidence rated limited and stronger for calcific deposits. We track your nights, not a success rate. Full rotator cuff page.

What To Expect

What happens at the first rotator cuff treatment visit for Wildwood patients?

01

Confirm it is the cuff

Night shoulder pain also comes from a frozen shoulder, the AC joint and the neck. Dr. Cheshire examines the shoulder and neck, tests strength, reads any imaging, and sorts worn from torn. New to the area? No local doctor needed yet.

02

Sleep set-up on day one

The pillow positions above, adjusted to your bed and your side, written down.

03

Weekly for about six visits

Needling first if the shoulder is too sore for anything else, shockwave on the tendon as it settles, laser where it fits, injections only if the tendon needs more. Home loading between visits.

04

Nights as the marker

How many times you woke, which side you managed. Then a taper. If the nights are not improving by visit four we re-examine and say so.

Is This You, Wildwood?

A strong fit when:

  • The shoulder wakes you or decides which side you sleep on.
  • Reaching overhead or behind your back is the worst of the daytime.
  • It has hurt for more than three months.
  • Imaging, if you have had it, showed tendinopathy or a partial tear.
  • You would rather rebuild the tendon than manage the pain indefinitely.

A surgical opinion first for sudden weakness after a fall or injury. Something else if the shoulder is stiff in every direction, which may be frozen shoulder, or if the pain runs from the neck down the arm. We screen for all three at the consultation.

Common Questions

What do Wildwood patients ask about rotator cuff treatment?

Lying on it compresses the swollen cuff tendon against bone; lying on the other side lets the arm hang and pull on it; and at night the sensitized signalling has nothing to compete with. Dr. Cheshire reviews your medications, including blood thinners, before the first treatment, and anything injectable is supervised by Dr. Phipps, MD.
On the good side with the bad arm on a pillow in front of you, or on your back with a small pillow under the elbow. A pillow behind your back stops you rolling onto it.
Usually it means a worn tendon, which is far more common. Sudden weakness after a fall is the sign that points to a full tear, and we screen for it.
Needling the guarding muscles usually changes the nights within the first two visits. Rebuilding the tendon takes the series.
Seventeen randomized trials, about 1,100 patients, pooled in 2024: less pain and better function against control, evidence rated limited. Stronger for calcific deposits. Source: Xue et al., BMC Musculoskeletal Disorders, 2024 (17 trials, about 1,100 patients). We state the limits of the evidence as plainly as the results, and measure your own progress from the first visit rather than quoting success rates.
About six, weekly, with home loading between, then a taper as sleep and reach improve. Progress is scored against measurements taken at the first visit, and the plan is changed or stopped if the numbers have not moved by the checkpoint.
Acupuncture visits are covered by many plans. Shockwave, laser and injections are self pay, and Medicare covers none of it for shoulders. We confirm before you book. Call 352-430-2720.
